In order to complete a successful adoption through China,
families must complete the necessary paperwork, known as the Dossier.
Families who submit an EAC
Preliminary Application for Adoption and are approved will receive the China Adoption Guide and
Registration Packet. This includes samples of the dossier forms and information needed in order
to complete the dossier documents. Families will also be assigned a personal China Adoption
Consultant to assist your family through the adoption process.

documents necessary to adopt through EAC.
China Dossier Documents
Adoptive parent(s) are required to have all dossier documents authenticated in
the manner required by the China government. Your EAC Adoption Consultant
will assist your family in completing 2 sets of the following documents. There can
be no white-out or cross-outs or corrections on any of these forms.
The following documents must be notarized* and certified:
1. Home study - with accompanying documents
2. Marriage certificates - certified copy
3. Birth certificates for each parent - certified copy
4. Divorce decree (if applicable) - certified copy
5. Medical statements
6. Employment letters - Non-employment statement if unemployed
7. Local police clearances on letterhead
8. Financial statements
9. Previous year's 1040 tax forms
10. Passport copies
11. Letter asking to adopt
12. Guardianship letters
13. I-171H, Notice of Favorable Determination from CIS
* Each document must be less than 3 months old, completed individually and have its own notary.
