In order to complete a successful Guatemalan adoption, families must complete the necessary
paperwork, known as the Guatemalan Dossier. Families who complete and submit the EAC
Application for Adoption will receive the Guatemalan Adoption Guide and Registration Packet. This
guidebook includes samples of the dossier forms and information needed in order to complete these
dossier documents. Families will also be assigned a personal adoption consultant to assist your
family in completing the necessary paperwork.
Guatemala Dossier Documents
Adoptive parent(s) are required to have all dossier documents legalized in the manner required by
their local State's requirement as well as the Guatemalan government. Your EAC adoption consultants
will assist your family in completing the following documents. Each family is to complete two sets
of documents - - - all must be originals. There can be no white-out or cross-outs on any of these
forms.
The following documents must be notarized* and certified:
- Marriage certificate (if applicable) – certified copy
- Divorce decree and/or death certificate (if applicable) – certified copy
- Certificate of health
- Home study report (singles must have a report stating they have a positive role model)
- Two declaration of witness statements per applicant
- Birth certificates – certified copy
- Name affidavits
- Local police clearance letters of good conduct
- Employment letters
An I-171H, Notice of Favorable Determination (cabled to Guatemala City,
Guatemala) is also required.
* Each document must be completed
individually and have its own notary. All documents must be less than 6 months
old once submitted to Guatemala.*
