In order to complete a successful adoption through Kazakhstan, families
must complete the necessary paperwork, known as the Dossier. Families
who complete and submit the EAC Preliminary Application for Adoption
will receive the Kazakhstan Adoption Guide and Registration Packet.
This guidebook includes samples of the dossier forms and information
needed in order to complete these dossier documents. Families will
also be assigned a personal adoption consultant to assist your family
in completing the necessary paperwork.

Kazakhstan Dossier Documents
Adoptive parent(s) are required to have all dossier documents legalized in
the manner required by the Kazakhstan government. Your EAC adoption consultants
will assist your family in completing the following documents. Each family is to
complete two sets of documents -- all must be originals. There can be no white-out
or cross-outs on any of these forms.
The following documents must be notarized*, certified, and apostilled:
- Cover letter (no notarization, apostille or page number required)
- Letter of intent to adopt a child in the Republic of Kazakhstan
- Parents Commitment Letter to Kazakhstan Adoption Law
- Home Study -1st as part of a dossier, 2nd separate copy with a short cover letter (total 2)
- Home Study Social Worker’s License
- Home Study Current (unexpired) Agency License
- Home Study Agency post placement commitment prepared by a HS Social Worker
- Home Study Agency letter of recommendation
- Int'l Adoption Agency "Certificate of Approval"
- Int'l Adoption Agency License
- Monitoring Commitment of Adoptive Family after adoption is finalized
- Post Adoption Reports Commitment from the family
- Consular Registration Commitment from the family
- Photocopy passport copies for each Prospective Adoptive Parent
- Marriage Certificate
- Birth Certificate of Prospective Adoptive Parent – for single parents ONLY
- Guardianship affidavit – for single parents ONLY
- Divorce decrees (if applicable)
- Single Woman Letter (if applicable)
- FBI Background checks separate for each Prospective Adoptive Parent
- Medical Forms for each of Prospective Adoptive Parents
- Employment letters, separate for each Prospective Adoptive Parent
- Financial Statements (Affidavits)
- Bank letter
- Home Ownership form of Prospective Adoptive Parents
- CIS approval of an orphan petition – USA
- Power of Attorney for International Adoption Agency Representative in Kazakhstan
- Power of Attorney for International Adoption Agency Representative in Astana, Kazakhstan
- Pictures of Prospective Adoptive Parents
* Notaries please note: Your commission cannot expire within a twelve month
period FOLLOWING your notarization of these documents.
All documents must be less than one (1) year old
