Russia Adoption Documents
The Foreign Dossier

EAC requires each family to complete three sets of documents; all must be originals. Below are the procedures to be followed as explained in the EAC Russian Adoption Guide & Registration packet (samples will be provided).

Visit the Russia Forms Library for all your non-government documents necessary to adopt through EAC.

    The following documents must be notarized and/or certified and apostilled:
  1. Marriage license / Birth certificate (if single*)
  2. Heterosexual Statement (if single*)
  3. Financial statement for Adoptive Applicant ODHS 1681
  4. Photos of adoptive family and home
  5. Home Study from Licensed Agency
  6. Home Study Agency's license (current)
  7. Social Worker's License
  8. Agency Letter of Authorized Services
  9. Consent and Approval Form
  10. Post Placement Verification Letter
  11. Local police clearance on letterhead
  12. Employment letter on letterhead (This can not be written by yourself personally. It must be written by employer or accountant.)
  13. Photocopy of Passports
  14. Powers of Attorney - must be typed
  15. Statement of Living Conditions
  16. Promise to Register with Consulate
  17. Promise to Register with Post Placement
  18. Copy of I-171H Notice of Favorable Determination, cabled to Moscow, Russia, DO NOT notarize the original, only copies.

ALL DOCUMENTS MUST BE LESS THAN 12 MONTHS OLD AT TIME OF SUBMISSION. Your notary's commission cannot expire within a 12 month period following the notarization of these documents. Some regions may require that police and employment documents be less than 3-6 months old. All medicals must be less than three months and will not be requested until a referral has been accepted as regions require different medical forms. Psychiatric evaluations may be necessary as well. EAC suggests you locate a psychiatrist in your area in the event that this is needed after the acceptance of a referral.

*Singles should include a letter from guardian in each file. Guardian must be in adoptive parents age bracket or younger (no grandparents). Letters should state the relationship to applicant as well as willingness to raise adopted child(ren) through adulthood if adoptive parent is incapacitated or in the event of death. Also, state that the guardian will provide an education, while also providing for the child(ren) emotionally, medically, and financially.
