EAC requires that each family complete two sets of documents, all must be originals. Below are the procedures to be followed as further explained in the EAC Ukrainian Adoption Guide & Registration Packet:

4.         Certified copies of Marriage License

5.         Medical Certificate

6.         Police Clearance Letters- State level, not local

7.         Home Study

8.         Home Study Agency License

9.         Social Worker's / Employment Confirmation Letter

10.       Social Worker's License

11.       Letter of Obligation

12.       Limited Powers of Attorney

13.       Deed to the Home

14.       Passport Copies

15.       I-171-H Immigration Approval
