Gathering Medical Information

      The following are questions that you can ask and seek answers to on your first trip:

             Birth History:
                     Birth Mother
                         Age, number pregnancy, gestation
                         Prenatal care or not
                         Disease history and treatment
                     Birth Father
                         Age, any other information available
             Delivery:
                     Delivery history, apgar scores, diseases at birth and treatment
                     Details on any resuscitation and hospitalization

             Hospitalization:   
                      Course, diagnoses and treatments

             Orphanage:
                      Dates of stay

             Growth parameters:
                      Height, weight, head circumference, chest circumference
                      Updates are very important
                      Babies every one to two months
                      Toddlers, children every two to three months

              Developmental milestones:
                      What is child able to do and how well
                      Sits, crawls, walks
                      With or without assistance
                      Feeds or dresses self
                      Talks, understands
                      Interaction with caretakers and other children

              Illnesses and treatments
                      Course, diagnoses and treatments

              Medical evaluations
                      Names, dates and results

              Immunizations received
                      Name, lot number and dates given

              Lab work
                      Name of tests done and dates of results

              Any other pertinent medical information that is available

 * It is suggested that parents in the adoptive stage between referral and first trip, keep a notebook to write any questions that they might have reguarding their child's medical information, etc. It can then be taken on the first trip and any answers given can also be documented.
